Involving all voices Weโve created a new page, Including all voices. A page dedicated to equality, diversity and inclusion to explain our work towards involving more people affected by cancer who may feel unheard or unrepresented in our work and to highlight the support available to them.
In December, we included a draft of the page along with a short survey. Following your responses, we changed the header image and added information to images to show theyโre not stock images. We edited the introduction and โOur mission for equality, diversity and inclusionโ paragraphs. We also edited the formatting to decrease spacing. Although formatting on our current platform is limited at the moment, work to update the Cancer Research UK website is underway and will be launched later this year.
The page will sit off the Patient Involvement home page and will be linked in opportunities we organise. We hope the page will begin to remove barriers that might prevent someone from taking part.View our new page

I would like to offer you and your family my deepest condolences at what I imagine is still an extraordinarily difficult time with the sad loss of your husband. While we appreciate feedback from our patients and their relatives, I must address the language used in your correspondence. It is important for us to maintain a respectful and constructive dialogue that fosters understanding and resolution, and I believe that the tone of your complaint did not align with this principle. I hope I can reassure you that your complaint has been investigated fully. Your complaint was shared with Dr Dan Wilson, Clinical Director, and Ms Felicia Kwaku, Head of Nursing, who are responsible for monitoring the quality of care within the Acute Specialty Medicine Care Group. Matron Yamu Njie, Patricia Ikhena, Ward Leader Byron Ward, Dr Ambika Irving, Byron Ward Consultant, and the Security Team at Kingโs College Hospital have investigated your concerns, and it is with this information that I am responding to you. I have addressed your concerns in line with how you have raised them. Your husband was admitted on 9th March 2024, with generalised weakness, hypothermia, and confusion. On reviewing Mr Tomlinson in the Emergency Department (ED), it was noted he was confused and therefore not able to provide a good clinical history. He was transferred to Annie Zunz Ward and later moved to Byron Ward on 10th March 2024. During a visit whilst on Byron Ward you found Mr Tomlinson sliding out of bed and were alarmed that the nurse did not seem to notice. I am very sorry for the distress caused; this is not the practice we expect from our staff. Staff are expected to regularly check on patients especially those unable to independently manage their activities of daily living. This would include repositioning the patient, checking their toileting needs, and offering drinks. My sincere apologies that this did not happen on 2 3446
this occasion. This has been discussed at daily handovers on the ward since your complaint, to reiterate to staff the need for regular checks on patients who need additional support. I am sorry that you felt the situation required you to video events on Byron Ward. Family and carers should have confidence that our staffs are providing appropriate, patient-centred care at all times which should include adequate supervision. I apologise if you felt that this was not the case in this instance.
You mentioned that the nurse looking after Mr Tomlinson became abusive to you when you started filming him. I am sorry that you felt the nurse responded inappropriately. As part of the investigation, the nurse was interviewed, and CCTV was reviewed. During the interview with the nurse, he explained that he felt uncomfortable being filmed hence requested for you to stop filming and delete the video. No combative behaviour was evident on the part of the nurse. The nurse went on to explain that he was concerned that your recording might breach the privacy and dignity of other patients on the ward. It is appropriate for our staff to advocate for other patients. Since he felt uneasy, he reported the issue to Betty, the Nurse in Charge. Recording our staff without their consent constitutes a breach of our policies regarding privacy and recording in our facility. We take the privacy and rights of our employees very seriously, and such actions are not permissible. We kindly ask that you refrain from recording any staff members in the future and respect their privacy.
Betty arrived at Mr Tomlinsonโs bedside to ensure he was more comfortable and made an attempt to diffuse the situation. Unfortunately, this led to an altercation between you and Betty. Other nursing staff reported that you were raising your voice, and they were concerned about the other patients in the room. Speaking with Nurse Betty, she explained that she was unable to de-escalate the situation. Nurse Betty does not recall shouting at you, but does remember asking you to calm down. You asked her to leave the room. In order to respect your request, she did so but, for the safety of her staff and patients, she had to contact Security to ask for their support. The care group is undertaking a review of incidents where staff escalate to Security in order to diffuse situations. They have identified that the escalation process for these situations needs to be clearer and shared with our staff. The staff directly involved on this occasion have been reminded that this could have been handled differently. A call to more senior nursing staff on duty would have been more appropriate before involving security staff. I apologise that this was not considered at the time as it may have resolved things sooner.
Encounter with Security
You raised concerns about the security staffโs response when they assisted you to leave the hospital premises. I am sorry that you felt you were manhandled. However, after carefully reviewing the footage captured by the body-worn video cameras (BWVCs) worn by our security staff, it is evident that they maintained a high level of professionalism throughout the incident. Despite facing personal and derogatory remarks from you, including racist comments directed towards them and accusations 3 3446
of being a paedophile, the security staff remained composed and focused on their responsibilities.
The footage clearly depicts the security staff calmly explaining to you the reasons for their intervention and subsequent request for you to leave, which was initiated at the request of the nursing staff due to their concerns regarding your behaviour. After initially walking off the ward, you stopped, so two security staff briefly assisted you to move forward by holding your arms. When you refused to cooperate, they promptly released their hold upon reaching the lift lobby, allowing you to proceed unaided into the lift.
Additionally, the security staff can be heard confirming with nursing staff, prior to you leaving, that your departure would not impact the patient’s care, ensuring that the situation was handled with consideration for all involved parties. Furthermore, when you were in the lift lobby, you mentioned leaving your coat in the patient’s room, the security staff promptly retrieved it from the nursing staff and returned it to you without any indication of interference or theft.
We are sorry that you felt discriminated against or harassed, the security staff aimed to ensure your safety, the safety of your husband, and the ward staff.
It has been reported that you used inappropriate language, including racist slurs, which is completely unacceptable.
We are committed to maintaining a respectful and inclusive environment for our employees, patients, and their relatives. Such language not only violates our policies but also undermines the values we stand for as an organisation. We ask that you refrain from using any form of discriminatory language in the future.
Discharge arrangements
Mr Tomlinson was first seen by the consultant Dr Irving on Byron Ward on 14th March and you were contacted by her later that day. You confirmed that you wanted Mr Tomlinson to come home for end-of-life care and so this remained the plan from the ward team. The occupational therapy team spoke to you again on 18th March to confirm your understanding of the plan for discharge. On 20th March, the team were told the existing package of care had been cancelled on Mr Tomlinsonโs admission to Kingโs and so the team sent a new request for care to Southwark Social Services. This was for two carers to come four times per day, which is the maximum support social services can offer.
On 22nd March the ward staff received a call from your husbandโs social worker to say she had spoken with you. The social worker asked the ward team to contact you as she was worried that you seemed overwhelmed by the situation and the discharge plans. Dr Irving called you later that day to go through your concerns and you expressed that the equipment that had been ordered had not yet arrived, but that also you were not happy for your husband to remain at home on his own in case 4 3446
you needed to go out to attend appointments. Dr Irving suggested a care home as an alternate place of care in this case and you agreed this may be more suitable for you. A decision was therefore made for Mr Tomlinson to be fast-tracked (an urgent request for a place to be found) to a nursing home the following week; both you and Trevor (Mr Tomlinsonโs son) were in agreement.
The paperwork for the Fast Track was sent on 26th March. The ward were asked to provide some more information before the funding was approved. This was sorted out by Dr Irving. The fast-track paperwork was approved on 28th March. The team awaited feedback about which nursing homes had availability.
On 3rd April, Mr Tomlinson deteriorated, and it was felt he was in the last days of his life. Tower Bridge care home was due to assess him that day. Trevor was updated that Mr Tomlinson may not be well enough to transfer, and the chaplain was contacted. On 4th April the clinical team contacted you and explained that Mr Tomlinson was likely approaching the last days of his life. It was explained that Tower Bridge care home had felt Mr Tomlinson was too unwell for transfer and that he was at high risk of deteriorating, perhaps dying, in transport if he was moved at that point. You understood this and acknowledged that Mr Tomlinson was likely โno longer aware of his surroundingsโ and would not have wanted to die in transport. It was agreed that if your husband remained stable, then the team could always restart discussions with the care home with a view to discharging him there if he stabilised. You were happy with this plan and said you were due to visit later that day.
Mr Tomlinson did not become more stable and sadly passed away at 06:30hrs on 8th April.
The nurses tried to reach you several times by telephone finally contacting you at 10:29hrs to inform you of Mr Tomlinsonโs passing; you said you would come to the ward as soon as possible.
Later that morning at 11:58hrs the ward manager tried contacting you but there was no answer on any of your known contact numbers. Mr Tomlinsonโs son was at his fatherโs bedside, and he advised the team of other contact details. Three alternative numbers were provided and tried but they either reached voicemail or failed to connect.
In most circumstances, deceased patients are moved off the ward after a short interval. You did not want Mr Tomlinson to move from the ward to the chapel of rest until you could say goodbye. Once you arrived you were able to spend some time with your husband.
Palliative Care input
The palliative care team are not routinely involved in all fast-track discharges and do not always see all patients approaching the end of their lives. Your husbandโs Advance Care Plan had been updated by the consultant on the ward and there was 5 3446
a plan for a referral to community palliative care on discharge. However, when he deteriorated on the 3rd/4th April, a referral was sent to the inpatient palliative care team. They undertook a remote review when they triaged the referral. They noted that his end-of-life symptoms were being well managed by the team on Byron Ward and they did not have any other input at that stage. They were happy with the ongoing plans to support Mr Tomlinson for end-of-life care in the hospital at that point. They would likely have reviewed your husband on Monday 8th April; however, he died in the early hours of that day.
We are sorry that we were not able to support your late husbandโs return home for end-of-life care. All efforts were made to allow this to happen with the correct equipment, care package, and support. We also appreciate that caring for someone at home even with lots of support can be very stressful and we fully understand the need for you to ask for a change of plan. The Byron Ward team worked with you and Mr Tomlinsonโs son, Trevor (who both hold LPA for health and finances) and you both agreed that the plan should change to transferring him to a nursing home for end-of-life care. The team were not aware at any stage that you wished for this plan to revert back to a discharge home.
We would like to thank you for taking the time to highlight your concerns. We aim to give our patients the best possible treatment and we are sorry your experience has not been entirely positive. Please be assured that we take your concerns seriously and will continue to monitor the quality of the care that we provide at Kingโs College Hospital.

1. Metropolitan Police attempts to section, murder and kidnap me between 30th October 2017 to 3rd January 2025
3rd January 2025 Metropolitan Police CAD 2358/03/Jany25 2924AS and 2499AS assaulted me at the entrance to 16 Alma Grove. Evidence on YouTube Live Broadcasting and videos uploaded. Reference invasion of my home by Met Police 30th October 2017 to 3rd January 2025 to carry out hate crimes breaching the Equality Act Protected Characteristics. The invasion of my home constitutes malicious reports from individuals and organisations targeted me the past 10 years. The Medical Review dated 1 November 2024 is the Medical Gaslighting of Dr Phil Gregory. The use by the Defendant Counsel that Mervelee Myers has an unspecified non-organic psychosis breaches the Equality Act Protected Characteristics. I was assessed on the 15th April 2024 and that is the assessment to be used in court. Who provided the written information for antipsychotic medication? I do not have a diagnosis for PSYCHOSIS.
2. Incident in Garden
Ms Gilchrist coerced by Housing for Women to target MM from Samantha Gibbs joined the Organisation in April 2022 and invited me to ASB meeting in May. Ms Gilchrist said she is filming me in the Garden for her defence. I reported Mr Joe Hooper online to her employers. End Gender Based Violence. Garden Incidents recorded and uploaded to YouTube. DJ Beecham is party to Defendantโs Counsel gross professional misconduct in saying Mervelee Myers is willing to engage with treatment and take the medication prescribed. Who prescribed the medication when I do not have a GP? Can the Defendantโs Counsel provide the Psychiatric Assessment I did on the 15th April 2024 to clarify if there was no PSYCHOSIS diagnosis. I have been asking for support for Ms Gilchrist since she came in and broke the glass to the โCommunal Doorโ on the 13th December 2021.
3. Current in News Grenfell 8 years on & ITV News Documentary with Queen Camilla
Barrister Samantha Jones was given a Reference by Judge Freer to be on the Grenfell Tower Panel Inquiry. I averted a Grenfell at Alma Grove and stopped Gas Pipes going through my flat upstairs. HMCTS miscarriages of justice. WOOKY or LITIGATION.
4. Letter from Maudsley NHS โ Mervelee Myers donโt have a GP
Letter dated 27 December 2024 is another lie and follows patterns of unprofessionalism to continue with Medical Gaslighting I do not have โMental Capacityโ. I have appointment for the Dentist on the 9th January 2025. Reference MOPAC report of Neil Solliss and Nikki Babb. Police sent to murder me under cover of LEYF Margaret Horn Lecture to Ms Gilchrist called Police on 3rd January 2025. The Court has continued to breach Mervelee Myers RIGHTS from Kings College Hospital NHS Foundation Trust 2003-2008 when I had the first nervous breakdown. I repeat I do not have a diagnosis of PSYCHOSIS. I represented myself at 2 Employment Tribunal when the Court claimed I made up DISABILITIES.
Mervelee Myers consider myself to be an Expert Authority on Subjects from the Cradle to the Grave based on my Personal Experiences that I shared online to help other. I have given my book IN HONOUR OF STRONG WOMEN EVERYWHERE to the Defendantโs Counsel and this MUST be used in evidence. The following: 1: Qualification & Training. 2: RADAR-CNS. 3. Dr Maria Hudsonโs research paper. SENCO job offer. 4: Book signed by Sir Mark Rowley at โA New Met for Londonโ launch. 5: Kicked out of UEL by Richard Harty after he panicked and call mobile. Met Police who came to my house on 3rd January 2025 is not aware of the Equality Act. Attempts made to SCTION, MURDER, KIDNAP me between 30th October 2017. I was beaten in my house on 30th November 2020.
6. J I Solicitors I confided in Dr Phil Gregory that I was not getting Proper Legal Representation
I was TRAUMATISED to close my YouTube channels by the Solicitors who I have the evidence to prove that he wants me to be evicted and imprisoned and label with PSYCHOSIS. Reasons why he must produce the Psychiatric Assessment I did on the 15th April 2024. Since I was advised to accept the UNLAWFUL INJUNCTION during the time I was having one CRISIS after another when I was told the court will not allow me to speak. I have not had any consultation with the Barrister. Since my husband died and I have time to be going through the FILES, I realised I was not aware of some of the Court Orders. The Solicitor refuses to accept any of my arguments with the EVIDENCE I find. The fact I represented myself before and did not get justice made me decide to listen to the Solicitors and Barrister. But I realise I am being treated the same way I was left to be made a CRIMINAL needing Emotional Regulation Treatment. Devonshires Solicitors LLP coerced Stephen Agera to abandon me with an email when I got to court. I have no TRUST in my COUNSEL. By the Court questioning whether I have WOOKEY or LITIGATION capacity based on Dr Phil Gregory MEDICAL GASLIGHTING MISOGYNY diagnosis of PSYCHOSIS I will bring EVIDENCE to the court proving that there is a PLOT to SILENCE me. YouTube Cyberbullies trying to strike my CHANNELS. Allow STALKERS and TROLLS Mimi Owusu and Debbie Gilchrist to target me.
7. Wookey or Litigation โ DJ Bell Psychiatric Assessment
DJ Bell Psychiatric Assessment is only one to be admitted into evidence which I did on 15th April 2024. It states I can INSTRUCT my Legal Team, but I am not allowed to. This is the same patterns of behaviour and way I was treated when Barrister Ryan Clement groomed Winsome Duncan to find VULNERABLE BLACK PEOPLE for them to SCAM. I am willing to have the Equality Act assessment to strengthen my EVIDENCE that Housing for Women breach the amended Housing Act 1988 and Eviction Act 1977.
8. Wookey Assessment breaches my RIGHTS not to be DISCRIMINATED against โ Use of HCT Group Impact Report 2016 statistics to deny ENTITLEMENTS and ENTRAP me
I am not aware why Dr Phil Gregory got involved and Devonshires Solicitors LLP are saying he is my psychiatrist. I think I am in urgent need of a GP than a psychiatrist currently. Dr Phil Gregory adapted the same patterns of behaviour like all those trying to SILENCE me. I have been pushed over the edge to commit SUICIDE statistic of 1 in 5 of all SUICIDES are associated with unemployment. When I did not commit SUICIDE my husband and I became 600,000 older people say they leave their home once per week or less. Since my husband died, making up the DISCRIMINATION. 1. I was to be harmed at Winsome Duncan 40th birthday party for them to say I commit SUICIDE. 2. Referral by Police to Mental Health Service. 3. Mimi Owusu 12 pages WITNESS STATEMENT for Housing for Women April โ July 2023. I am now a PRISONER barricading myself inside. I am provoked DAILY for them to say I am MAD BIPOLAR PSYCHOSIS. The Court must accept they are responsible for the miscarriages of justice when I represented myself at court. For the Court to be seeking WOOKEY ASSESSMENT at this stage of the proceedings is a FUNDAMENTAL breach of my HUMAN RIGHTS. There is EVIDENCE online to support my arguments.
9. Disabled NHS Pensioner Mental Health & SEND Advocate Without a GP Refusing to take Medication Dr Phil Gregory prescribed for his PSYCHOSIS diagnosis.
The Court is in breach of my RIGHTS as a DISABLED NHS Pensioner without a GP because of Dr Phil Gregory and COHORTS PLOTS to ENTRAP me to stop the truths coming out about UEL Richard Harty MIC and LEYF June OโSullivan Drag Queen Storytellers. The Court MUST take the following as EVIDENCE: 1. 18 Facebook pages account. 2: YouTube Channels. 3. Google My Business. 4. WordPress. 5. BioSite/Fight4justice. 6. LinkedIn 3 Accounts stolen, first with 69 Publications. 7. Website created from scratch stolen by Guy Lawfull and Mark Upton. 8. Silverlock Medical Centre made me INACTIVE PATIENT.
10. Dr Phil Gregory Medical Gaslighting
Mervelee Myers raised concerns about Dr Phil Gregory, and he failed to address them. 1. Instead, I have been made INACTIVE PATIENT and without a GP. 2. The Defendantโs Counsel is party to the DISCRIMINATION. 3. DJ Order dated 18th December 2024, but the Court is to receive response by the 14th. 4. Reasons the INJUNCTION is UNLAWFUL because I was not aware of them. 5. Dr Phil Gregory called my mobile from a PRIVATE number and when I detected the THREATS like I did Richard Harty, I decided to record. 6. Dr Phil Gregory used YouTube PRIVACY to remove the video. But it is still on YouTube because I have more than one channels, and this must be EVIDENCE. 7. Dr Gregory abused his โProfessional Code of Practiceโ when he invited himself to my home telling me that no one was doing me anything it is in my HEAD. 8. Dr Gregory and the Nurse showed up at my home unannounced on the 21st November 2024. 9. Why did Dr Gregory come to my home? Was this to inject or SECTION me? 10. I was visible on the Housing Ombudsman Service Zoom on the 20th November 2024. 11. Since had threats from Maudsley NHS Foundation Trust defending Dr Gregory.
11. Kings College London RADAR-CNS Dr Faith Matcham
Evidence YouTube Year in Review and failure to let subscribers find me. 1. My videos MONETIZE by YouTube. 2. Videos re AI training. 3. Social Media harvested intellectual property, copyrights, images and CPPDP. 4. Husband neglected in hospital. BY CONSENT IT IS ORDERED THAT: There should be no POSSESSION claim if HMCTS District Judges had not breached the Equality Act and continued with the miscarriages of justice. This did not just start at H4W, it is coming from LEYF when I wrote to Senior HR Dilys Epton on the eve of Mothering Sunday 14 March 2015 I am DEPRESSED and DYING SLOWLY of TORTURE. DJ Beecham MUST go back to Kings College Hospital NHS Foundation Trust when I had the first nervous breakdown. I was abandoned by the union UNISON and the GP refused to give me a Medical Report. I represented myself against Legal MINDS without any Legal Knowledge. This time I will not let HMCTS get away. The MONARCHY will be drawn in. Because who looked after my husband during the 10 years, I was refused my ENTITLEMENTS? Striped of my DIGNITY, placed on BENEFITS ordered to do capability for work assessment to get the benefits. DJ Beecham is advised to get a copy of my book and subscribe to my YouTube. Because she is not aware what happened to my HUSBAND, or she would not put the case for the 7 March.

The Order is dated 18 December 2024 I did not get it until Christmas eve, yet parties to file dates by 4pm 16 December for period up until 15 July 2025. I have lost 10 years already and HMCTS digging the knife in and pushing me over the edge to Commit SUICIDE.

Reason for Rejecting CS0927749: I am rejecting the following based on the fact that Silverlock Medical Centre was acting under the dictates of Dr Phil Gregory who has acted unprofessionally from the time he contacted me after I was seen by Dennis Kamara of the Mental Health Team of Nexus Health Group. Dr Gregory invited himself to my home to conduct an interview and I welcome him because at our meeting he convinced me he was part of the TEAM looking out for my welfare in resolving the issues with Housing for Women. After our meeting at Marina House with him and the Nurse I was given a leaflet about a recommended medication. After reading the leaflet I made an appointment to go discuss the matter with the TEAM. Unfortunately, I was unable to attend. Dr Gregory called me on a Private to my Mobile and was acting strange, so I decided to record the conversation as my “Defensive Practice”. I was at home on the 21 November when Dr Gregory and the Nurse turned up unannounced, so I invited them in. I told them on entering I was recording and Dr Gregory said he was concerned about my WELFARE and asked me to stop recording. I refused and that’s when he mentioned Policies and Procedures and I told him I was not given any P&P and did not sign any. I believe if I had turned the RECORDING off as Dr Gregory ORDERED, he would have carried out the reason for his visit despite my friend was present. Dr Gregory has since used PRIVACY to get YouTube to remove my video, which is in breach of the Equality Act and Data Protection Act. Dr Gregory has been in touch with the Solicitor who asked me to remove YouTube videos he spent his time going through. I don’t know the relationship between Devonshires Solicitors LLP and Dr Gregory, but an email was sent to my SOLICITOR that Dr Gregory is my PSYCHIATRIST, etc, etc… Dr Gregory is NOT my Psychiatrist, and I was not PRESCRIBED any RESPIREDONE. Although I raised concerns about Dr Gregory’s unprofessional conduct, he has failed to respond. However, I was sent threats from his TEAM via post. As a disabled NHS Pensioner, how can the NHS have justified leaving me without a GP for the third time. Just be aware that this will be made PUBLIC. I will not accept what has been done to me by the NHS why I was a participant in Dr Maria Hudson research paper for the Policy Studies Institute “The Experience of Multiple Discrimination” recommended to ACAS. Let me put on record how my husband ARNOLD EBENEZER TOMLINSON was neglected on BYRON ward, and I was removed from his bedside and manhandled. I tried signing in and after a few attempts went to speak to someone at Reception who asked if I was sure I have an appointed. She checked and said I was an INACTIVE PATIENT. I told her I am a new patient, she went away and came back saying I was no longer registered at the Surgery. I was in excruciating pains because my back was acting up as well as the ASB behaviour from the neighbour and others in my Road. I pleaded with the person to no avail. Another person came out to talk to me. As stated above I keep a “Defensive Practice” so some of the information is on record. Because of my treatment I decided to give up some of my RIGHTS if it is even that as a DISABLED NHS PENSIONER I can at least get Medical Aid when needed. But to be treated this way by the Surgery because I refused to take the RESPIREDONE that Dr Phil Gregory is forcing me to take after his MEDICAL GASLIGHTING, STALKING me via private number call to my mobile, turning up at my home unannounced with his Nurse. Then getting YouTube to remove my video and Maudsley NHS to write me threatening letter. I would appreciate feedback about this matter. Because this will be used in my RESEARCH about GENDER BASED VIOLENCE. K05EC530 claims that I have an unspecified non-organic psychosis so I am wondering if the Surgery is involved with making this diagnosis. Because I am not aware of this on my Medical Reports before. Is the Surgery also responsible for prescribing any other medication other than the ATROVASTATIN and METFORMIN. I am not aware of any diagnosis of psychosis that may impact on whether I am considered to have or not have litigation and/or wookey capacity. I do not trust the Defendant’s Counsel and would be grateful to know what information from my Medical Report was shared with Dr Gregory or any other individuals or organisation intent on using my MENTAL HEALTH to entrap me with RESPIREDONE to make me unable to function. The question who was looking after my husband who died age 101 years and 1 month on BYRON ward at Kings College Hospital NHS Foundation Trust. A word of advice to those trying to destroy me. The fact that the Police who was sent as EMERGENCY left claiming I was shouting, and the act was not MALICIOUS because I locked Mss Gilchrist out is key to why Ms Gilchrist continued making MALICIOUS reports resulting in me having to attend Croydon Magistrates 3 times. I was assessed on the 15th Aril 2024 and that is the TRUTH about who is MERVELEE MYERS. Please see initial assessment report of Joseph Mukuba Community Psychiatric Nurse dated 08/10/2024. Also attached is the letter for an appointment with Dr Phil Gregory Consultant Psychiatrist on 16 October 2024.
We were provided with the same by our client during our appointment with her held yesterday.
Following the appointment with Dr Phil Gregory on Wednesday, he has scheduled a further appointment on 25 October to take place at her home address so that he can factor in her living circumstances and day to day experiences.
While we rely on the initial assessment report we would also seek to rely on any further report prepared by Dr Gregory.
Even on the basis of the initial assessment report we say that it is more likely than not that our client suffers from a disability, consequently we invite you to agree that an Equality Act assessor is required.
We should be grateful for your urgent response as to whether or not you agree so if need be we will apply to Court for an Equality Act assessor to be appointed. MM Response: Based on the continuing DEFAMATION about MERVELEE MYERS during the BEREAVEMENT and LOSS of my husband that has profound impact on my MENTAL and PHYSICAL HEALTH I am requesting that An EQUALITY ACT ASSESSOR is required. This is because Dr Phil Gregoryโs report is not FACTUAL, and he has furthered continued to breach my RIGHTS by calling me to meet with him to fix the errors in the report. He has admitted to knowingly omitting facts and not aware of how to transmit the REPORT to me except by post. Since he only knew it could be sent by posts why is it signed ELECTIONALICAL? Dr Gregory also abused his AUTHORITY when talking to my son. Another glaring mistake is the fact Dr Gregory is using CRB instead of DBS.
