On 13/8/21 I took my Disabled Student’s Allowance Disability Evidence Form to Decima Surgery. I discussed it with the Receptionist who advised me to speak to the Nurse. The Nurse said it was the job of the GP. I left the form with the Receptionist, who said it should be ready within a week. I was advised by Dr Laura Crawford of www.healthmanltd.com to seek counselling to find out why I react the way I do to certain situations.
I was going to the Dentist on 19/8/2021 and stopped to find out if it was ready. The Receptionist searched for it, said there were no records of it being left.
